Missed Appointments
An $85.00 fee will be assessed for a missed appointment. A cancellation with-in 48-hours of your scheduled appointment is treated as a missed appointment and a $85.00 fee will be assessed. This fee is allowable, appropriate, and necessary since insurance coverage does not allow for reimbursement of missed appointments. The 48-hour advance notice will allow the appointment time to be offered to another person.

Late Cancellations and No Shows A late cancel is an appointment cancelled without 48-hours notification and is considered a missed appointment. If this occurs on two consecutive instances, your appointment time will be reviewed to ensure attendance.

A no-show is defined as non-attendance without 48-hour notification is considered a missed appointment. If this occurs, it is required to provide notification of desire to continue services with-in 24-hrs of missed appointment. If this occurs for two consecutive appointments; appointments will be paused until no show fees are paid and communication of continued desire for services is provided. In the absence of confirmation to continue services and/or payment of fees it will be accepted as a request to terminate psychotherapy.

Reminder Calls This office provides reminder notifications via text messaging as a courtesy, notify the front office to opt-out.

Credit Card Processing Fees Payment made with a credit card are assessed a 3.5% bank convenience fee. Payments made with a debit card (pin must be entered), cash, or check are not assessed a fee. Visa and MasterCard is accepted.

Records Requests Fees for records are $0.50 per page up to 50 pages, then $0.25 each additional page, and a search/handling fee of $20.00, plus shipping costs.

Social Media
To maintain confidentiality and to prevent dual relationships acceptance of friend requests or contact requests on any social networking is not appropriate.

Telehealth Services This includes the use of such technologies as telephones, facsimile machines, electronic mail systems, store-and-forward technologies, and remote patient monitoring devices that are used to collect and transmit patient data for monitoring and interpretation.
